I have the same issue sometimes, and I believe it's indeed diaphragm control. When I manage to get it all out at once it's because I successfully got to use my diaphragm to create pressure in the belly during the release. I find breathing "with your belly" helps doing that, which is when you breath in air to your lungs but expand your belly instead of your chest. I know singers use that to be able to breath and sing properly and I think it's related to that usage of the diaphragm. But I find hard to master it and create that pressure for my burps, though they have definitely improved over time,so I feel I'm on the right path.

Ok, I can't at all scientifically explain this, but concerning the control problem, this is what I do - You can inhale and hold it in and it will escape whenever.
However, when you inhale, don't close your pipes, just keep them exactly as if you were to inhale - but exhale.
I hope this helps just a little bit.
I tend to have more control when I just keep the motion going.
